Medical Home Model Merges Patient-centered Care with Tradition
The Yukon-Kuskokwim Health Corporation (YKHC) provides healthcare for a region roughly the size of New York—approximately 58,000 square miles— and includes a network of 50 village health clinics, five sub-regional clinics and the Yukon-Kuskokwim Delta Regional Hospital (YKDRH). The population consists of 58 federally recognized tribes, which suffer from high rates of infectious diseases, dental disease, cancer, death due to injuries, and mental health issues.
A unique setting and care delivery model is required to align with traditional healing and environmental conditions. The Paul John Calricaraq-YKHC Clinic is a welcoming, comfortable, and culturally relevant environment that dramatically improves access, quality, customer satisfaction and ultimately, the health of the residents of Southwest Alaska. Healthcare Performance Partners led the design process through the development of workflows for patients originating in the village setting and ultimately being cared for at the YKHC Clinic. The workflow design then informed the development of the clinic layout to support the unique needs of the patient.
The foundational operating concept is a Patient-Centered Medical Home with each patient assigned to a care team, or medical home. Each home provides an integration of services to form a comprehensive system of personalized care.
By focusing on the wellness of the whole person in the ambulatory clinic setting, the patient care model ensures an increase in value-added activities and elimination of non-value added activities.
